Community Connectors is a free, confidential service for people over 60 or those caring for someone who is.

We aim to connect older people and their carers to information, local services and activities to support them to live as well as they can for as long as they can. We focus on helping older people and their carers recognise and build on their own strengths, to increase their wellbeing and to live the life they want to live.

Recognising that everyone’s journey is unique and that no two peoples’ needs are the same, we have four distinct but interconnected levels of support from which our clients can choose their own package.

WHAT DO WE OFFER?

 

One to One, Person Centred Support

We will spend time getting to know you and what matters to you. We can work together to create a plan to help you take the next steps including tailored lifestyle management plans.

For those who need a little extra support, we can liaise with other service providers on your behalf to ensure you get the support you need to live the life you want.

  

Signposting & Information

We can provide information on local activities, services and groups to suit your individual needs.

We can work to increase your social networks or help you access support to improve your health and wellbeing.

 

Volunteering Opportunities

We can provide information on volunteering opportunities in your local area and support you to find the role that’s right for you.

 

WHO IS THE SERVICE FOR?

The service is open to anyone over 60 or anyone caring for someone over 60 in the following areas:

North West

G3, G4, G11, G12, G20, G22 and parts of G13, G21 & G23

North East

G4, G31, G32 and parts of G69 (south of M8 only)

South

G41, G42, G43, G44, G51 & G52
